We are always committed to providing you with the best dental care. Our commitment combined with your help allows us to succeed in our goal of excellent dentistry promoting good dental health.
We start by offering you a complete initial examination including necessary X-rays. Our diagnosis and treatment plan comes as a direct result of these services. We will review our recommendations with you and discuss the total cost to complete your chosen treatment plan. At this time PRIOR to treatment, we will offer financial arrangement options. We offer comfortable choices including credit cards and interest free financing to approved applicants. We always stress communication and feel it is very important to discuss fees before treatment begins.
With this in mind, we leave no questions unanswered and you will leave our office each visit satisfied we kept our commitment to your good oral health and you have kept your commitment to pre-arranged financial plans and cooperation in keeping all appointments.
IF PAYMENT ARRANGEMENTS HAVE NOT BEEN MADE OR APROVED IN ADVANCE, PAYMENT IS EXPECTED AND DUE IN FULL AT TIME OF SERVICE.
FINANCIAL POLICY REGARDING PATIENTS WITH DENTAL INSURANCE
Our policy remains the same regarding insured patients. There is usually a co-insurance and deductible due regardless of the type of insurance you or your employer have purchased.
Definition of DEDUCTIBLE- The amount of dental expense the PATIENT is responsible for
BEFORE the insurance will make any payment of benefit.
Definition of CO-INSURANCE-A provision in your contract whereby the patient shares in the cost
of covered services.
Please remember YOUR INSURANCE IS A CONTRACT BETWEEN YOU AND THE INSURANCE COMPANY. YOUR EMPLOYER PURCHASES THE CONTRACT IN HIS BEST INTERESTS. WE ARE NOT A PARTY TO THAT CONTRACT. We will submit all necessary forms to your insurance carrier. Most times payment will be made to the Doctors but your deductible and coinsurance must be calculated and paid as service is rendered in compliance with our financial policy.
WE MUST EMPHASIZE AS DOCTORS OF DENTAL HEALTH, OUR RELATIONSHIP IS WITH YOU, OUR PATIENT, NOT YOUR INSURANCE COMPANY. WE ARE ALWAYS EAGER TO HELP YOU RECEIVE YOUR MAXIMUM ALLOWABLE BENEFITS. In order to achieve this goal we need your assistance. YOUR INSURANCE CARD MUST BE PRESENTED TO US AT YOUR FIRST APPOINTMENT OR YOU WILL BE RESPONSIBLE FOR PAYMENT IN FULL AT TIME OF SERVICE . If you cannot provide us with your correct dental insurance information, you will be expected to make payment in full at that time. It is also YOUR responsibility, as the patient, to provide us immediately with any insurance changes.
PAYMENT OPTIONS
We have several helpful payment options available to offer and assist our patients with their dental treatment plans.
- Our office accepts Mastercard and Visa credit cards and personal checks.
- After initial consultation, if treatment plan exceeds $1000, any patient who wishes to pay in cash in full at the start of treatment will be given a 5% discount on the total treatment fee.
- We work with our own finance company to be able to offer any of our qualified patients a six-month or one-year INTEREST FREE financing option depending on the total treatment cost.
